PBH occasionally collects information through our Web site. We may collect the following information about visitors: Name, Address, Phone & Email. This data is not required, and is collected only requested. This data is collected at the following locations:

PBH may also collect information [Name, Address, Phone & Email] which our users divulge about other people, such as additional attendees.
PBH collects this information through online registration forms for specific events. (i.e. Conferences & Courses offered by PBH). The information is collected voluntarily. To attend PBH conferences & events, Name, Address, Phone & Email is mandatory. In order to have an accurate count, and/or contact the registrant should the event change. Other information requested is optional.
PBH collects general data [survey results, forum data] that is not personally identifiable information.
PBH does employ cookies. A cookie is a small text file that the Web server places on a user’s computer hard drive to be a unique identifier. Cookies do not have an expiration date. PBH cookies do not collect personally identifiable information. Cookies can be disabled by the user at any time.
Use of Personal Health Information (PHI)
PBH is committed to ensuring the privacy and confidentiality of our members’ PHI. We collect and disclose such information only according to our strict confidentiality policies and federal and state laws designed to maintain privacy according to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) privacy rule. The PBH Notice of Privacy Practices explains how we use information about our members and under what conditions we are permitted to share that information with others.
The information collected by PBH will be used for registration purposes (for example; conferences and/or training). Users who provide information may receive email announcements about upcoming events, trainings, opinion polls & conferences. Out of respect for the privacy of our users PBH presents the option to not receive these types of communications.
The information collected may be used to create customer profiles based on opinion pools and/or event history.
PBH offers links to other Web sites. Please note: When you click on links to other Web sites, we encourage you to read their privacy policies. Their standards may differ from PBH’s.
PBH’s Web site offers chats, forums and message boards. Users are advised that information posted on these venues becomes public knowledge.
The PBH website may offer Anonymous Health Screenings for all users. The data is reviewed by the PBH Medical Director for statistical purposes only.
