A decrease of a provider or practitioner level may occur if a provider or practitioner fails to meet any of the criteria outlined by PBH identified on the Profile Decrease Criteria Grid. If a provider or practitioner encounters a situation meeting the criteria for a level decrease, the PBH Network Cross-functional Team reviews the information and make a recommendation for a level decrease evaluation to the PBH Network Advisory Council. The PBH Network Advisory Council reviews the recommendation and supporting documentation related to the level decrease. The PBH Network Advisory Council makes a decision regarding the appropriate level decrease utilizing the Performance Profile Decrease Criteria Grid. Providers or Practitioners are allowed to attend the PBH Network Advisory Council to present any relevant information to the Network Council members. The decision of the PBH Network Advisory Council is final.
