Hospital Staff:

  • If you are planning discharge of a consumer to a nursing home, adult care home or family care home:

GAST staff members are familiar with many of the long term care facilities in the 5 county area we serve. We may be able to provide information to hospital discharge planners about the strengths and capabilities of various long term care facilities for discharge planning situations.

We can also proactively offer specific training or consultation to facilities that we have been informed will be accepting a person discharged from a state facility.

  • If you are planning discharge of a consumer to home to be cared for by a family member or friend:

Should the caregiver need education to better understand the needs/behaviors of the consumer, GAST can provide service in the form of education or consultation to the caregiver, either before or after discharge of the consumer. Remember, in these cases the consumer must be age 60 or above. If the consumer is less than 60 years old, to qualify for GAST services, he/she must have “geriatric-like” needs, for example early onset dementia.
