Gold Star Provider/Practitioner Rating System

The PBH Provider Performance Profile is a leveling system designed to encourage providers to adopt innovative, quality practices that help connect consumers/family members with important information about services, resource availability, and quality performance.   PBH will monitor Provider and licensed independent practitioner Performance Profiles based on Quality Assurance activities, national accreditation, provider self-report, consumer report, domains for risk management, non risk management, quality performance, and recommendations from consumer satisfaction reports, including items on the state FEM tool.   The following information outlines the process used in determining the placement of Providers in the PBH Network on the Performance Profile, as well as maintenance of the level obtained, and criteria for a decrease in level.
