| Was informed that individual no longer received community support, instead only peer support. QP stated that she signed generalized service plan, but has never met individual and was not required to meet indivdual to develop service plan. Stated she received her information from peer support worker. QP refused to give credentials when asked. | |

| Turning Point Services does not provide adequate staff to meet my child's needs and give him the hours he recieves from his innovations waiver. Our one home support worker we had quit without a notice and Turning Point did not provide backup staff and said it could take weeks to find someone to provide services for my son. | |

| I have been seeing a staff member at outlook behavioral health for more then six months now. All of the treatment that I have recieved has been outstanding. I would always suggest this provider for my friends and family or anyone needing help. | |
