• Initial Placement on the Performance Profile

A new contracted provider and/or practitioner would enter the PBH Performance Profile system on Provisional Status until active service provision occurs with PBH for 90 days. The provider would be eligible for advancement to Routine status after successful completion of the 90-day Service Implementation Review and achievement of a minimum score of 75%. Routine Provider/Practitioners are reviewed annually by Quality Management and are not eligible for advance placement on the profile system until service provision has occurred with PBH for one year. Routine is defined as “meeting compliance based standards only”.
